Referral Agents

 

This information is for those who would like to make a referral to the service:

When you first make contact with the Befriending Service to refer a client, we will complete a Referral Enquiry Checklist which will assist us in making the right decision about your client.

If your enquiry meets our criteria, a full Referral Information Form will be sent out for you to complete and return to us once you have seen the client in person so that consent can be obtained before sharing the information with us. Where possible the client should also sign the form.    

Following completion of the Referral Information Form we will conduct a Home Visit Assessment. The home visit will be arranged at a mutually convenient time with the client and may involve you or a representative being asked to accompany our staff on the visit.

The purpose of the visit is to help us to ask questions which will assist us with matching your client to the right befriending service we provide and make a suitable match. 

Following the Home Visit Assessment the referral will proceed to our Case Management Meeting which takes place at 8 week intervals after which we will be in touch with you and/or you client to discuss options.

There are a number of Befriending Service options which we manage and deliver through our Befriending volunteers. Some take less time than others to set up and each have different criteria which we will discuss with you when you phone.
